2症例目は金沢大学医学類6年の稲垣さんが、リウマチ・膠原病内科で診療された多発関節炎と四肢末梢の浮腫を主訴とする75歳女性についてプレゼンされました。関節リウマチ、Remitting Seronegative Symmetrical Synovitis with Pitting Edema (RS3PE)症候群等の鑑別を中心に診療が開始されましたが、入院経過中に悪性リンパ腫とそれに伴う血球貪食症候群が顕在化し、最終的には悪性リンパ腫に伴うRS3PE様症状としての関節炎、浮腫として化学療法によりそれらの症状が改善した症例でした。プレゼンの中で、関節リウマチの分類基準について言及されることがありましたが、ティアニー先生は「分類基準で関節リウマチが診断されるのではなく、私が診断したものが関節リウマチである」とまさに内科医の頂点としての矜持を示されました。
ティアニー先生はアメリカで老舗の内科教科書である「Current Medical Diagnosis and Treatment」の主幹編集者を2008年まで務められました。2004年～2005年頃にかけては松村先生が当時勤務されていた石川県立中央病院（関連病院参照）での勉強会に多くの医学生や若手医師が詰めかけ、このCurrentを持ってサインの列に並んだものでした。その医師たちは今や全国の教育機関・医療機関で主軸となって活躍しています。ティアニー先生の診断理論や珠玉のパールは松村先生の訳で世に出ています。

また、学会開催に先んじて、9月27日にWilliam E. Rainey先生（University of Michigan, USA）に金沢にご来訪いただき、原発性アルドステロン症の歴史から、最新の知見に至るまで、多岐にわたる内容のご講演をいただきました。なかでも、原発性アルドステロン症の最初の症例を報告されたConn先生の肉声をお聞きすることができたことが非常に印象的でした。
