各国の特色が反映された発表が並ぶ中で、ポスターセッションで注目を集めたのは当研究室、山田先生の発表されたCKD stage G3~4の患者群に於けるトルバプタンの有効性についての報告でした。(The usefulness of TOLVAPTAN in patients with ADPKD with CKD stage G3 to G4)
自分自身は、透析低血圧は定義によって関連する危険因子が異なり予防的介入をする上で混乱を生ずるという問題提起が主旨の発表でした。(Impact of intradialytic hypotension definitions on the factors related to IDH) 今回が初めての海外学会参加であり、データの解析・ポスター作成で諸先生方の助けを頂きやっと辿りついたものでしたが、その先に「世界の腎臓内科医は何に注目しているのか」「どのようなプレゼンテーションなら注目を集め、メッセージを伝えられるのか」というこの先目指すべき目標を示された思いでした。
各医局が医学部時代の名称に回帰するなか、私たちは教室の特性をシンプルに表し、市民の皆様にとっても分かりやすく親しみを持って頂けるよう、「循環器病態内科学」と致しました。英語名は「Department of Cardiovascular and Internal Medicine」とし、循環器領域のみならず、内分泌代謝、腎・リウマチ、消化器の各分野において幅広く、深く内科学を追求していく決意を新たにしているところです。
今回は日本からは2名しか参加者がおらず、ヨーロッパ開催ということでほとんどの参加者が欧州から来ておりました。参加者はだいたい50名程度でした。初日にBrugada三兄弟の一人であるJosep. Brugada先生による心電図の講義があったのですが、心電図1枚を提示して、参加者の一人が英語でプレゼンテーションをするという形式のものでした。今回非常に名誉なことなのですが、Brugada先生に指名を受け、50人の前でプレゼンテーションをする機会を頂きました。途中までは問題なかったのですが、英語を聞き逃し、癖で”Yes”と言ってしまったところ、”Oh, you are yes man !! Yes means that you don`t know anything. OK?”と言われてしまいました。実際、日本人は具体的に反論したり、意見を言ったりする機会が少ない事、英語力の問題からYesと言ってしまいがちですが、それ以降の議論に繋がらないためこのような態度は避けるべきだと痛感させられました。間違える事よりも意見を言わない事の方が評価を下げるそうです。3日間みっちり講義を聞き、クタクタになりましたが、心電図や心内心電図の細かな見方をしっかり学ぶ事ができ、非常にためになりました。

要約:近年、日本人の脳梗塞の主要な原因である心房細動(AF)は新たな国民病とも言われています。その原因は加齢、高血圧、心不全、心筋症などがよく知られていますが、一部の孤発性心房細動(lone AF)は家族性であることが明らかになっています。本論文は、lone AF 90人に遺伝子解析と機能解析を行い、8人(8.9%)にAF発症に関与する遺伝子変異を見出しました。更にその一部にはイオンチャネルの機能亢進変異が認められており、これらの症例に対してはイオンチャネル阻害薬が有効と考えられます。今後の創薬に繋がる重要な研究成果と言えます。
